The About Your Donation card type displays a field set on the Donation object as a collection of editable fields. The card type uses a data source to create a new donation record and when users submit the page, the values they entered in the editable fields are populated on the donation record.
The field set must be on the Donation object and the data source must return a Donation sObject, like the NewDonation Data Source.
